Political Sub-Divisions Annual SBE/MBE (Set-Aside) Program Compliance
The State of Connecticut DAS/Supplier Diversity forms and instructions are available below. The Department of Administrative Services has provided the Annual Goals Calculation Report Forms to you in Excel format. This will allow you to complete these forms in a convenient and expeditious manner. Also, you will be able to electronically send these forms to your DAS' Suppler Diversity Program Business Development Representative, Stan Kenton, at stanley.kenton@ct.gov as well as a copy to CHRO's Representative, Lydia Mendez, at lydia.mendez@ct.gov .

Please return your Political Subdivision's completed Quarterly Report as an e-mail attachment to: Stanley.Kenton@ct.gov and Lydia.Mendez@ct.gov.
The Quarterly Reports are due as follows:
1st Quarter Report (July 1 - Sept. 30) DUE OCTOBER 31
2nd Quarter Report (Oct. 1 - Dec. 31)* DUE JANUARY 31
3rd Quarter Report (Jan. 1 - Mar. 31) DUE APRIL 30
4th Quarter Report (Apr. 1 - Jun. 30) DUE JULY 31

* Pursuant to C.G.S. Sec. 4a-60g, section (m), “any state Agency or Political Subdivision of the state, other than a municipality, that achieves less than 50% of its small contractor and minority business enterprise set-aside program goals by the end of the second reporting period in any twelve-month period beginning on July 1 shall provide a written explanation to the Commissioner of Administrative Services and the Executive Director of Human Rights and Opportunities detailing how the agency or political subdivision will achieve its goals in the final reporting period."
