Veteran’s Documentation Required


1. If you’re applying for credit as a VETERAN (5 points):  Proof of Honorable Separation showing BOTH dates of entry into and separation from the service.  Acceptable proof includes a certified or photostatic copy of one of the following: DD214 (long form) or Department of Defense certificate of honorable separation showing both entry and separation dates.

2. If you’re applying for credit as the SPOUSE OF A VETERAN NOT GAINFULLY EMPLOYED DUE TO DISABILITY (5 points) or UNMARRIED SURVIVING SPOUSE OF A VETERAN (5 points):  In addition to the proof listed in number 1, the following must also be submitted:  Marriage Certificate and a statement from a physician certifying that your spouse is unable to pursue gainful employment due to disability.  For those applying for unmarried surviving spouse of a veteran credit, the death certificate of your spouse or an official notice of death if it occurred in the line of duty, and a signed, dated statement attesting that you have not remarried since the death of your spouse are required.

3. If you’re applying for credit as a CURRENTLY DISABLED VETERAN (10 points):  In addition to the proof listed in number 1, the following must also be submitted:  Statement from the Veterans Administration dated within the last six (6) months, certifying that you are currently eligible for compensation or pension benefits.

4. If you’re applying for credit as the SPOUSE OF CURRENTLY DISABLED VETERAN NOT GAINFULLY EMPLOYED DUE TO DISABILITY (10 points) or UNMARRIED SURVIVING SPOUSE OF A DISABLED VETERAN (10 points):  In addition to the proof listed in number 1, the following must also be submitted:  Marriage Certificate, a statement from a physician certifying that your spouse is unable to pursue gainful employment due to disability, and a statement from the Veteran’s Administration dated within the last six (6) months, certifying that your spouse is currently/or was eligible for compensation or pension benefits. For those applying for unmarried surviving spouse of a disabled veteran credit, the death certificate of your spouse or an official notice of death if it occurred in the line of duty, and a signed, dated statement attesting that you have not remarried since the death of your spouse are required.
